In the plot, Stella Grant has cystic fibrosis, a disease that affects her lungs and keeps her trapped in a hospital for most of her life: so that no infection compromises the possibility of a lung transplant that can give her a few more years of life, she needs to keep six steps away from any sick person.
Will Newman, for his part, only wishes to be 18 years old soon to be able to get rid of his fibrosis treatments, since after being contaminated by B. Cepacia, an indestructible bacterium, he lost all chances of receiving a new lung and has already accepted the fact that he will die soon. It is exactly all that Stella needs to avoid in order to be successful in her transplant, but for them six steps is not a safety issue, it is a punishment.
